解決陰性染色的問題非常簡單，就是設(shè)立“陽性對照”。如果陽性對照有了表達，說明染色的全過程和所有試劑都沒有問題。如果此時測試片仍為陰性，便是真實的陰性，說明組織或細胞沒有相應(yīng)的抗原表達。反之，如果陽性對照沒有著色，表明染色過程中某個或某些步驟出了問題或試劑出了問題。應(yīng)一一尋找原因。陽性對照包括兩種，一種稱為“自身對照”或“內(nèi)部對照”，這是指在測試的切片中本身就存在已知的抗原，如正常淋巴結(jié)中存在T和B細胞抗原，CD20或CD3都應(yīng)該有表達。自身對照是一種比較理想的對照，對照和測試組織或細胞都在同一張切片中，都處于相同的試驗條件下，結(jié)果更可靠也更具有可比性。在選擇自身對照片時選擇既有病變組織同時又有正常組織的部分，這樣有利于對比。另一種稱為“外部對照”，有時在測試的切片中不存在已知的抗原，如在胃的標(biāo)本中懷疑是惡性黑色素瘤，需要用HMB45或Mart-1來檢測，在正常的胃組織中本身不存在相關(guān)的抗原，如果病變出現(xiàn)陽性反應(yīng)結(jié)果，尚能提示是惡黑，但是如果出現(xiàn)陰性結(jié)果，就無法確定是本身組織中不含黑色素瘤抗原，還是技術(shù)問題。因此，應(yīng)另外設(shè)立一個已知的陽性對照。這種在測試組織之外的陽性對照稱為“外部對照”。在實際工作中需要設(shè)立外部對照的情況很多，如果每一種抗體都要選不同的陽性對照，工作量會很大。為了解決這個問題，目前國內(nèi)外有單位將多種不同組織集成在一起，制成多組織切片、“臘腸”“春卷”切片、組織芯片等，其連續(xù)切片儲備待用，需要時取出一張便可作為陽性對照。另外，比較簡單的方法，是采用闌尾作為陽性對照，因為與人體其它組織器官比較闌尾包含的組織種類較多，如有上皮、淋巴組織、平滑肌、間質(zhì)、神經(jīng)、間皮、血管等。一張闌尾切片可以檢測大多數(shù)常用的抗體。
